Indicate an accident or malfunction with hazardous substances
If an accident or malfunction or illness or death related to hazardous substances has occurred in your company, you must report this to the competent authority.
You must notify the competent authority of an accident or malfunction in activities involving hazardous substances if there has been serious damage to the health of your employee(s). You must also report cases of illness and death if there is a concrete suspicion that they were caused by the activity with hazardous substances.

You must notify the competent authority of an accident or malfunction by e-mail or post:
- You submit your ad.
- The competent authority will examine your notification.
