A civil partner who bears a civil partnership name may, after dissolution of the civil partnership (dissolution or death of the other civil partner), resume his/her maiden name or the family name used until the civil partnership name was determined.

  • Current certified copy of the civil partnership register, if the register is not kept at the registry office where the declaration is made
  • Final divorce decree or death certificate
  • Valid identity card or passport

Hints

The declaration can be made either at the registry office of residence or at the registry office where the civil partnership was established.

In the case of a declaration at the registry office, this will forward a certified copy of the declaration to the registry office that certified the civil partnership at the time.

The declaration of re-admission becomes effective when it is processed at the registry office where the civil partnership was established.
