If your driver's license has been lost or destroyed, for example due to theft, illegibility or loss, you will need a new driver's license unless you surrender your driver's license. If you do not immediately report the loss of your driver's license and apply for a replacement document, you are committing a misdemeanor.
If your name has changed, you do not have to get a new driver's license. However, especially when driving abroad, it may be useful to be in possession of an EU card driver's license with current information. If you wish to change your name on your driver's license, a new card driver's license will be issued upon request.
