Informal notification with the following information:

• Company address
• Address of the business premises or the construction or assembly site at the event location
• Date of event
• Type of event e.g. explosion, media leak, crash
• Number of injured persons, number of killed persons
• Equipment involved
• included approved monitoring body (for systems requiring monitoring according to Annex 2 of the Ordinance on Industrial Safety)
• involved body for determining the cause (for work equipment according to Annex 3 of the Industrial Safety Ordinance)
• Brief description of the event and the damage that occurred (including photos and videos) measures already taken by the employer

The competent authority may also request the following information from you:

• Risk assessment
• Proof that a qualified person has prepared the risk assessment
• Information about the responsible persons
• Information on protective measures taken
• Evidence of regular training of employees
• Operating instructions
